In compiling your tax data, we request that you gather the following documents and send them
to us via email, fax, U.S. mail, or FedEx.
If you cannot locate the documents, we will discuss what needs to be done via email or over
the phone.
Form W-2
Self-Employed business income and expenses
Form 1099
Form 1098
Form 1095's A, B and/or C regarding medical insurance
Closing statements from the purchase, sale or refinance of real estate
Form K-1's from LLCs, Partnerships, S Corporations, Estates, or Trusts
Brokerage statements regarding the purchase and sale of any investments
A copy of your prior year tax returns (new clients only)
Did you have Qualified Health Care Coverage (Minimum Essential Coverage) for you, your spouse, and anyone you could or did claim as a dependent
Information regarding foreign assets-bank accounts, trusts, retirement accounts or gifts
Any childcare expense information
Any other tax documents that you have a question about or that you have not summarized
Copies of current driver's licenses for e-filing
Copies of social security cards and confirmation of legal names for Tax Returns
